When filling out the card

We are ready to start collecting cards. Please fill in the card below. Once you fill in the fields there will be a pop up “accept and pay” button from our payment provider Braintree (a PayPal company). Complete the transaction and you will recieve an email from Adobe asking you to verify your signature. The card will then be sent to the UPW for countersignature. The countersignature is a manual process. We will try to complete the process within 1 to 2 business days and you will then receive a confirmation with the completed card and records in PDF form. These records will serve to prove our collection of cards when we apply to the C.I.R.B..

Instructions: This web form works best on a computer or tablet. If using an iPad try Landscape mode (sideways) to conveniently view the check boxes and fields. You must accept cookies from Adobe (“customize” or “enable all”). 

For the “Address” box, use comma separators for the address information (eg. street, city, province, postal code). Once the fields are completed look for a button or checkbox that says “agree and pay”. Click (or tap) on that and you will be taken to payment processing.

***Important***  Once you complete the process and payment you will receive a confirmation email, you must click on the confirmation link to complete the process. When that is done the UPW will get an email to counter-sign and you will receive your final copy. This may seem elaborate but allows us to track every card for presentation to the C.I.R.B..

 You can also apply by mail. Simply print out the card below (two pages). Fill in your information and send, with a cheque for $6.00 made out to the  Pilots’ Advocacy Society, to the address below.
